Risking Hyperstimulation

Man, I sure do wish I could bottle estradiol. It would be lovely to syphon some of that junk out of my body right about now. I finally got my E2 levels from the past three visits.

Friday, December 27- 29
Monday, December 30- 120
Friday, January 3- 1143
Monday, January 6- TBD

Now I didn't think 1143 was that bad, I mean it's higher than some people, but it's not like it's 4000 or heaven forbid, 6000. But I think the panic set it at the rate of it rising. Research seems to show that if the Estradiol levels more than double in 2-3 days, the risk for Ovarian Hyperstimulation Syndrome is elevated... Well mine significantly more than doubled (more like 10x) in 4 days. So yeah, needless to say Dr. Duffy wasn't super pleased.

At this point he is predicting a lupron trigger (rather than an hCG trigger) on Wednesday with a retrieval on Friday. He's hoping that I can stim at lose doses tonight and tomorrow night. Other options are to trigger earlier (tonight or tomorrow night) or coast a day or two without meds. I am fine with all of the above except the lupron trigger. Dr. Slayden had previously predicted the need for a lupron trigger (he was thinking 20-30 eggs), but he said that he would do a freeze all with a lupron trigger because pregnancy rates are lower after lupron triggers. Another doctor I saw during the hunt for the right clinic agreed with that... freeze all if we need lupron. Dr. Duffy doesn't seem to agree. He wants to do a lupron trigger and then give me just a bit of hCG on the day of transfer for luteal/pregnancy support.

Obviously, I have A LOT to research today... I absolutely do not want to transfer after a lupron trigger is it will jeopardize the success of the transfer. But I also hate the idea of doing a freeze all. Blughhh... If anyone has a method of extracting Estradiol from my blood, I'm all ears.

Now my Estradiol may be skyrocketing, but my follicles are not. There still hanging out at about a 1-2mm growth rate per day... sad trombones.

For comparison, here is Friday's ultrasound results:

Right ovary- 7, 9, 9, 9, 10, 10, 10

Left ovary- 6, 7, 9, 10, 10, 10, 10, 10, 10, 11

Lining- 10mm

And today's results:

Right ovary- 14, 16, 14, 15, 13, 12, 11

Left ovary- 17, 15, 16, 15, 14, 15, 8, 9, 16, 13, 17

Lining- 10mm

Now Dr. Duffy said that he'd like to reach 18-19mm for most of them before triggering, which would require another 1-2 days before triggering. Prayers for a lowish E2 level today would be much appreciated! I would really like to get the four 15mm follicles to 18mm before triggering.

So that's about it for me. I have less than 36 hours until Sam gets here... I cannot wait! This is the longest we've EVER been apart (slightly pathetic, but whatever) and I'm ready for him to be back in on the action! I'll update with the E2 results and the plan later! Oh, and hyperstimulaters: what were your E2 levels right before triggering?

  3. I know that Dr. Google is the worst! But from experience, OHSS is miserable. It is good that they are looking into Lupron. I used Ovidril as a trigger and within a day or two, I was a bloated mess that had to spend time in the hospital...plus a cancelled IVF. My estradial levels were at 5500 the day of trigger. I am praying for you though. So much of these journeys are up and down. I look forward to the update but just know that the doctors are doing what is best for you. Prayers your way!

  7. My E2 level right before trigger was 5,030! So I'm surprised that they went ahead with the transfer. Of course as you know I had to deal with hyperstimulation after retrieval and then again when the babies implanted and started producing HCG. It wasn't fun so be careful!

  8. I dont recall my levels before trigger. I did raise the question about using a trigger that did not have an HCG and they told me if they saw that was the case they would let me know or lower my dosage for my HCG shot. I'm guessing my levels were way low because they didnt seem to concern with it. My nurse even said I doubt you will get OHSS. HA! My e2 was a creeper bc I definitely started feeling the symptoms after the shot. Its not a fun thing to deal with. Best of luck! Hoping that is not the case for you.

  9. Hey sweetness. That is quite a jump ... But I don't think 1100 is too high in general ... Maybe your body will "even out." (Totally scientific, I know. ;) I am praying your estrogen hasn't jumped much further for today. Hopefully we've kept it at 1150-2000. I never knew that about an all-Lupron trigger, though I've never heard of a solely Lupron trigger anyway. I did a Lupron-HCG trigger once ... which gave me my first implantation. I hope you can still do HCG. Ohhh goodness, I know it's INSANE! I'm so glad Sam will be there soon. Hang in there ... P.S. I've never hyperstimmed, but my estrogen has gotten to 4000 twice ... if that helps. :/ In general, start drinking Gatorade; it will help sooo much hyperstim or not. :) xoxo...

  10. I don't have any experience with a Lupron trigger but my advice is to trust your doctor. There's a reason you chose him vs the other doctors you met with. What was you E2?!?! XO

  11. I wish I was a bit ahead of you in my cycle so that I could tell you not to worry about the lupron trigger. :( As much as it would suck, a lupron trigger and freeze all might be better than the alternative. It's a tough call. Thinking of you!

  12. No experience here - my E2 wasn't over 1000 until just before retrieval - but sending lots of good thoughts (low in E2!) your way. Glad that Sam will be there soon.

  13. I don't know much about E2 trending and how fast it increases each day, other than it increases with each follie that's contending and gives an idea how mature those follies actually are. I think you're worrying yourself unnecessarily. My E2 was 2814 day before trigger and 3681 day of trigger, and I was not hyperstimulated.     You can see my follies were not at 18 & 19 at last u/s and my E2 was much higher than yours too. Hope this provides some reassurance. I think you are going to have an awesome retrieval of nice mature eggs! I think your progression continues to look outstanding. Freeze all or fresh transfer, you are going to have plenty of embryos to work with!

  14. I don't have any experience with Lupron triggers, but all that information sounds overwhelming. I debate doing a freeze all for the next IVF, provided there are some to freeze. Praying that you get some good numbers for retrieval!

  15. Sounds like they are growing pretty steady. I was not always given my E2 levels, so I'm sure how the numbers correspond. Hope everything is ready to go soon!
